During the COVID-19 pandemic, the antimicrobial stewardship module in our electronic medical record was reconfigured for the management of COVID-19 patients. This change allowed our subspecialist providers to review charts quickly to optimize potential therapy and management during the patient surge.

A computer program was developed to analyse the relative amount of EMG activity in an agonist-antagonist pair of muscles while subjects performed voluntary flexion-extension movements at the wrist to track a visual target. The data were presented to the subjects in the form of a vector display, the angle and length of which was determined from calculation of EMG power in the two muscles.
This new approach to EMG biofeedback was evaluated in two hemiplegic patients and three patients with cerebellar incoordination. Over a training period of several weeks, all the subjects were able to modify the pattern of EMG activity in the muscles to reduce the amount of inappropriate coactivation of flexors and extensors and to produce more sustained and regular activation of individual muscle groups.

Morphological speech zone asymmetry in man cannot be due to environmental or developmental factors after birth. The functional implication of such a finding is not yet clear. Morphological asymmetry of the human brain is paralleled by electrophysiological evidence of cerebral hemispheric asymmetries. The results of our analysis of 50 infants suggest that clear occipital-temporal coherency asymmetry similar, but not identical to the adult pattern, also exists at or near birth. These asymmetries are generated by stimuli with no verbal content and in infants who presumably have no or an undeveloped capability for language. It is suggested that language is only a part of much more fundamental asymmetries which include the processing of auditory and visual information. Our results, and those of others, are consistent with the assumption that the left hemisphere is more able to relate stimuli to past experience, either short or long-term, while the right hemisphere is more able to process stimuli which are not easily identifiable or referable. These capabilities would not be based on language, and hence would be expected to develop independently and possibly before speech. The demonstration that reversing electrophysiological asymmetries can be generated with non-speech stimuli in the visual and auditory modalities, and in neonates, supports such an assumption.

Summary
Introduction
In this chapter, we discuss how social and biological studies of ageing can converge to provide a meaningful framework for progress in both understanding ageing and dealing with it in a positive manner. We start by discussing the meaning of the term ‘ageing’ and how it is in part defined by social context, and then, how psychosocial factors have an impact on both perception and the biological reality of ageing. From a theoretical perspective, we assess how ageing might have evolved, and how it is measured. The biological impacts of ageing are then described, moving from individual cells through tissues to major organ systems (immune, cardiovascular and nervous systems) (see Figure 2.1). What causes individual cells of the body to age is dealt with at both a cellular and molecular level, and we further discuss how studies of both extremely long-lived and short-lived humans have contributed significantly not only to our understanding of the biological processes of ageing, but also to the possibility of developing therapies to deal with the problems that cause greatest loss of quality of life in older age. We end by assessing the ethical case for intervening in those biological processes underpinning the development of those illnesses that so undermine health in later life.
Given the enormous scope and breadth of material that is covered, and the wide differences in perspectives and language used by the diverse disciplines that contribute to this chapter, we have tried to avoid jargon terms wherever possible, and provide simple definitions of unavoidable terminology as notes at the end of the chapter to assist the reader not specialist in that particular field.
Not ‘what’ is ageing, but ‘how’?
In order to study ageing in any meaningful way, we need to understand how the term ‘ageing’ is being used. To biologists, it can mean damage to molecules of the cell, and to cells of the tissue; to the physiologist, alterations in organ function; to the clinician, increased frailty and accumulation of diverse diseases. For the older person, ageing is felt and experienced, with changes in physical abilities and social activities and status having both positive and negative effects on the quality of their later life. Ageing is thus not so much a thing, but rather multi-dimensional, underpinned by complex social and biological processes, made up of many different mechanisms.

Risk factors associated with the presence and severity of food insecurity in rural Honduras

Objective
To identify factors associated with the presence and severity of food insecurity among a sample of Honduran caregivers of young children.
DesignCross-sectional study in which the dependent variable, household food insecurity, was measured using a fourteen-item questionnaire developed and validated in a population of similar cultural context. A predictive modelling strategy used backwards elimination in logistic regression and multinomial logit regression models to compute odds ratios and 95 % confidence intervals for food insecurity.
SettingRural Honduras in the department of Intibucá, between March and April 2009.
SubjectsTwo-hundred and ninety-eight Honduran caregivers of children aged 6–18 months.
ResultsNinety-three per cent of households were classified as having some degree of food insecurity (mild, moderate or severe). After controlling for caregiver age and marital status, compared with caregivers with more than primary-school education, those with less than primary-school education had 3·47 (95 % CI 1·34, 8·99) times the odds of severe food insecurity and 2·29 (95 % CI 1·00, 5·25) times the odds of moderate food insecurity. Our results also found that child anthropometric status was not associated with the presence or severity of food insecurity.
ConclusionsThese results show that among the sociodemographic factors assessed, food insecurity in rural Honduras is associated with maternal education. Understanding key factors associated with food insecurity that are unique to Honduras can inform the design of interventions to effectively mitigate the negative impact of food insecurity on children.

Insulin-induced hypoglycaemic response and release of growth hormone in depressed patients and healthy controls

As part of the Collaborative Study of the Psychobiology of Depression, we have examined the pretreatment growth hormone response (ΔGH) to insulin (0·1 U/kg) and the magnitude of the hypoglycaemic response in a large number of well-defined depressed patients (N = 132) and healthy controls (N = 80). After applying rigorous exclusion criteria, data were analysed from 93 patients and 66 controls for blood glucose response and from 56 patients and 52 controls for ΔGH. Depressed patients, either unipolar or bipolar, showed less of a fall in glucose than controls. A weak association was found between the magnitude of the fall in glucose and the severity of depression. No significant differences were found in values for ΔGH between the unipolar or bipolar depressed patients and controls either for males, pre-menopausal or postmenopausal females, or the total female group. These data do not support previous claims of a lowered ΔGH response to insulin in depressed patients. However, the resistance to hypoglycaemia seen in the depressed patients is consistent with previous reports.
Corrugated PVDF bimorphs as tactile sensors and micro-actuators – A research note

This note describes a new class of devices, corrugated polyvinylidene fluoride bimorphs. It describes the fabrication, testing and analysis of one simple configuration and discusses the general potential of the devices. As actuators, they offer promise for applications involving relatively large amplitude displacements and small forces with ease of control. As tactile sensors, the devices offer considerable promise due to their potentially high resolution, selectable compliance, sensitivity and durability.
